The character was most likely bitten by one of the poisonous creatures in wasteland (like common [[Radscorpion]]). The character will receive a small amount of damage over time until the poison will be expelled out of the body. The rate of receiving damage depends on the amount of poison inside the body. 

High [[poison resistance]] reduces the time a character is being poisoned. 
Poison can be cured with an [[antidote]].
