Lilly in the Vault City Headquarters will ask you to hand out questionnaires to the citizens. You can hand them out to citizens (in the bluesuits) or their servants, although handing a questionnaire to a servant will get you a negative response from Lilly when you complete in the quest. You have 12 hours in-game to complete the quest.
